• Strictly by appointment only. Upon claiming a coupon, My White Card customer care will call you within 24 hours to secure your preferred dates of appointment.
• If you wish to cancel your schedule or not use your coupon, click the remove button on your claimed coupon.
• Printing of coupons from the website or mobile app is strictly prohibited and will not be accepted by the Partner clinic. Once the coupon has been claimed, it will automatically be available in your claimed coupon page and will be validated and approved by the chosen clinic.
• Present 1 valid ID upon arriving at the clinic. Present your coupon through the My White Card mobile app.
• Coupons are non-transferrable. It may not be used as a gift to family, friends, or any other individual.
• Coupons are not valid in conjunction with other promos including Senior Citizen discount, PWD discount, and other promotions.
• You must be an ongoing My White Card member for the duration of the packaged services to claim and avail packaged coupon discounts.
• One Time coupons can only be used once for each My White Card account.
• Not valid for cashback. Non-refundable.
